Faith Formation Registration

The form below is for everyone seeking to register for faith formation (religious education) with Saint Rose of Lima. That means preparation for 1st Communion, Confirmation, and even RCIA (full initiation). And you only need one form for multiple children!
Once you submit, your registration is not complete until you drop-off donation & documentation to the parish office.
Donation is $80 for first child and $40 for each additional child.
Documentation: Birth certificate & Baptism certificate (& 1st Communion certifcate if enrolling in Confirmation).
The parish office is open Mon-Fri 8:30-12:30, 1:30-4:30.
