Baptism Application Process

Dear Parents,

Congratulations!  In presenting your child for baptism, you are accepting the responsibility of training him/her in the practice of the faith.  The parish community proclaims and celebrates that faith by keeping the memory of Jesus in the bond of the Spirit to the praise and glory of the Father.  We welcome your child's presence among us.

This web page has been developed to help you through the Baptism application process.  Please note the following requirements for parents and Godparents:

• Parents must be registered (active & participating) at St. Rose Church for at least 6 months

• Parent(s) and Godparent(s) are required to attend baptism preparation classes

• Parent(s) and Godparent(s) must be practicing Catholics

• Both Godparents, if married, must be married in the Catholic Church

• Both Godparents, if married, must turn in a copy of their marriage certificate with the paperwork

• Godparents must meet the Qualifications to Serve as Sponsor/Godparent at Baptism

Please complete and sign the Baptism Application Form and the Sponsor Form(s) and bring them to the St. Rose Parish Office, along with Godparent(s) marriage certificates if applicable, as one complete packet.
